what are we looking for?
Providing individualised and personalised care to clients that promotes their independence, enabling them to live and age well in place.
Providing consumer directed care services to our clients as prescribed in their care plan, including but not limited to:
o Personal hygiene and grooming(incl toileting and bathing),
o General housekeeping duties andmeal preparation
o Assistance with mobility andtransfers
o Transport to appointments andshops
- Actively participating in careplan development to ensure the clients' changing needs are being met
- Recognising and responding to change in a client's condition, ensuring this information is documented and communicated
- Engaging with clients and providing social support during visits
Selection criteria:
- Relevant Cert III i.e. Individual Support, Community Service, Aged Care
- Preferably 12 months experience in the Home Care sector
- Genuine empathy with elderly clients and a passion to work within the community service industry
- Willingness to undertake domestic care duties as part of their role
- Outstanding customer service and communication skills
Conditions of work include NDIS Worker Screening, Blue Card, open drivers licence, First Aid & CPR certificate, influenza vaccination and fully insured vehicle.
Hourly rate: $30.11 – $31.04 p/h (+25% loading for casual positions) salary packaging opportunities and kilometre-reimbursement offered for use of own vehicle.

Working collaboratively with clients and key stakeholders to provide comprehensive assessments and individualised case management. Care Partners play a key role in promoting the clients’ goals and wellness outcomes.
- Conduct Home Care Package client assessments and reviews, and provide comprehensive case management
- Maintain high quality assessment and care planning documentation
- Foster respectful relationships with clients to fully understand their needs
- Work collaboratively with community organisations, allied health providers and service agencies
- Manage client feedback and complaints regarding service delivery and undertaking remedy actions to address any issues
Selection criteria
- Preferably Diploma in Community Services / Health / Aged Care and/or Clinical background
- Minimum of 3 years’ experience in the Aged Care industry
- Demonstrated understanding of the Aged Care reform and Aged Care Quality Standards (ACQS)
- Experienced in case management with advanced knowledge of person-centred care
- Has experience in service development, driving continuous improvement, practice and service excellence

Salary: $71,000 - $85,000 per annum depending on skills and experience.

Providing safe, reliable, and friendly door to door transport service. As a community transport driver you will collect clients from their home and provide them with a safe and comfortable ride to their destination. But you are so much more than “just” a driver.
- Always maintaining a professional relationship, while demonstrating genuine care for our clients
- Following schedules and run sheets and keeping accurate records
- Liaising with schedulers and office team members
- Reporting observationsespecially if a client's needs have changed
Selection Criteria
- Has outstanding customer service skills and a love of interacting with clients
- Has excellent knowledge of the local area and is highly organised with great time management skills
- Has exceptional driving skills, and proven record of safety;

As a Community Support Partner, you would be providing support in a variety of ways to our wonderful clients. This may include providing domestic support in their homes, or transport and social support.
- Safely transporting clients to appointments or various other locations
- Attending and supporting clients during social group events

- Assisting our clients with basic food preparation and cooking
- Accompanying clients to the shops and assisting with shopping activities
- Documenting and communicating relevant information to colleagues and in our systems

- Conduct client assessments and reviews

- Provide comprehensive case management, monitoring the wellbeing of our clients
- Regular reporting, maintaining documentation and recording feedback / complaints.
Selection criteria:
- Minimum Cert IV in Community Services/ Health / Aged Care and/or Clinical background

Salary: Full-time $63,000 - $66,000 per annum
